The concept of All on 4, was introduced 20 years ago by Prof. Paolo Malo at his Malo Clinic, and later patented in collaboration with Nobel Biocare, a leading manufacturer offering scientifically proven implant solutions with a guarantee.

LOWER COST

The All on 4 system requires the insertion of only 4 implants. All on 6 or All on 8 systems require 6 or 8. Fewer implants mean lower costs.

In All on 4 systems, the two lateral implants are placed under angle. Because of this, it is possible to place specially designed Nobel Speedy implants. They can withstand a higher load, ie. more crowns.

GET TEETH THE SAME DAY

All on 4 implant placement system allows the patient to place temporary or permanent hybrid bridges on the implants the same day (hybrid work is a fixed prosthetic work consisting of crowns and artificial gingival-pink ceramics that imitate gums).

The hybrid work itself is made before the implant intervention, so that at the time of the insertion, it is already in the polyclinic. Hybrid work is adapted to the placed implants and fixed with micro screws. After the completion of the surgical part, the therapy is followed by prosthetics.

In the All on 4 technique, the contact area between the lateral implant and the jawbone is larger. The jawbone is used the best way possible, and the need for its compensation is reduced to a minimum. All on 4 allows us to avoid expensive and complex methods of bone replacement such as sinus lift.

With methods where the implantation of 6 or 8 implants is required, it happens that in some regions there is not enough bone available for implantation, so it must be compensated to a greater or lesser extent.

All on 4 is a technique that has been successfully applied worldwide for over 20 years, with a study of 16,000 patients, which showed a success rate of 98% over a ten-year level. It follows a clearly defined procedure, and implies usage of Nobel Biocare implants developed for this technique. All on 4 technique enables the patient to get fixed teeth that cannot be removed, with only one surgical intervention, which completely restores function and aesthetics, all in one day.

The concept is designed for patients who have lost their natural teeth due to periodontitis and are doomed to wear total dentures, which at best can compensate for a maximum of 40% of function and are very uncomfortable to wear. Sometimes wearing a total denture causes pain due to the nerve position, which is closer to the surface because of the bone resorption. Total dentures can speed up the process of bone resorption due to the pressure transmitted to the bone. The All on 4 concept enables the placement of a fixed bridge that does not come off, on four implants, and it completely restores 98% of the function.

All on 4 technique is intended for patients who must remove all teeth, patients who wear total dentures and the ones who have a limited amount of bone, i.e. insufficient bone.

A specially designed NobelSpeedy or NobelParallel implant is placed because it is the only one that is made to correspond with the All on 4 concept. The design provides high stability and has non-standard lengths up to a maximum of 25 mm (while ordinary implants have length of 16 mm), which allows the dentist to obtain the highest quality bone for implant placement. This bone is usually located under the nose because that is how all the chewing forces are transferred from the bridge to the bone. In this way, each patient gets the opportunity for rehabilitation and a new smile.

The whole process takes between 2 and 2 and a half hours. If there are existing teeth, they are removed, 4 implants are installed (which lasts about an hour) and a hybrid bridge with 12 teeth is immediately placed, which completes the part related to surgery (another hour and a half). During this short period, there is less stress on the tissues and the postoperative period itself is easier. After 4 months, a final work is made (a definite bridge that can be ceramic or made from other types of materials), which completes the whole process.

The placement of dental implants is an absolutely painless intervention. It is performed using local anesthesia.

There is no pain during recovery, while patients define the feeling after the intervention as mild discomfort.

The implant placement is a procedure that lasts from 45 min to 1:15 h. Thanks to the speed of work, the period after placement is very comfortable for our patients, without pain and swelling.

Today, very few conditions prevent the implantation of dental implants. These are autoimmune diseases or those patients that require organ transplantation. Heart disease is not a limitation for implantation and does not affect the life of the implant. Well-controlled diabetes does not interfere with implantation. Consumption of tobacco is a relative contraindication, however, the patient must be aware that smoking affects the life of the implant. It is not recommended for patients with uncontrolled diabetes and those who are in the phase of chemotherapy or radiotherapy. However, after some time, these patients can also have implants. For example, if a patient has severe diabetes, he needs to treat it and lower the parameters within normal limits. Then a hyperbaric chamber is added, which improves tissue blood circulation, because in diabetics in this case, the problem is not sugar, but the deterioration of peripheral blood vessels that are most important in the process of bone implants. After the end of this therapy, implants are installed and fantastic results are achieved.

If patients come from abroad and do not have much time, we can start making teeth, for example at 9 am and finish at 6 pm, thus completing the procedure completely. If the patient is still in the country, the sutures are removed within 7 days, and if he is not, the sutures that can fall off by itself are placed. After 4-6 months, the patient comes again to a control check-up, and then a definitive bridge is made, which can be metal-ceramic or titanium acrylate, and this completes the process.

PROCEDURE

The placement of dental implants is no more complicated than other oral surgical interventions. Before the implantation, the dentist analyzes the dimensions of the available bone on a 3D image of the jaw and decides on the type and dimensions of the implant. The procedure is performed under local anesthesia. After the procedure is completed, crowns are placed on the implants (bridges, All on 4, All on 6, All on 8) or they are covered with mucous membrane when there is a need to provide more time for osseointegration.
